About This Settlement

A class action lawsuit alleges that a cyberattack detected by Esse Health in late April 2025 resulted in the alleged unauthorized access of patients' personally identifiable information and protected health information — including names, addresses, dates of birth, Social Security numbers, health insurance information, identification numbers, and certain health information. Esse Health, which operates as American Multispecialty Group Inc., denies any wrongdoing but has agreed to a $2,525,000 settlement to resolve these allegations. Who Qualifies

• You may be eligible to file a claim if you meet the following criteria:
• You were potentially impacted by the Esse Health data incident that occurred in or around late April 2025
• You were sent a notice by Esse Health informing you that your private information was potentially compromised during the data breach
• No proof of purchase or additional documentation is required to claim the cash payment
• The settlement is nationwide — there are no geographic restrictions within the United States

What You May Receive

If you submit a valid claim, you may receive an estimated $50 cash payment. Please note that this amount is pro rata and may vary — either higher or lower — depending on the total number of valid claims filed. In addition to the cash payment, you may optionally enroll in two years of CyEx Medical Shield Complete identity protection services, which includes up to $1 million in medical identity theft insurance coverage. No payout amount is guaranteed.

How to File a Claim

1. Visit the official settlement website at EsseHealthSettlement.com to access the claim form.
2. Complete the claim form with your required personal information as requested.
3. Note that no proof of purchase or written explanation is required to claim the basic cash payment — simply submit a valid claim form.
4. If desired, opt into the two-year CyEx Medical Shield Complete identity protection enrollment as part of your claim.
5. Submit your completed claim form by the deadline of August 4, 2026. Claims submitted after this date will not be accepted.
