What You May Receive

Class members who submit a valid claim may receive one of the following benefits:

• Up to $5,000 in reimbursement for documented out-of-pocket losses directly related to the data breach, such as unreimbursed expenses for credit monitoring, identity theft recovery, or other related costs — documentation is required to support this type of claim • A pro rata cash payment estimated at $75 for those who do not submit documented losses, though the actual amount may vary depending on the number of valid claims submitted • Two years of medical monitoring benefits, available to all eligible class members

Actual payout amounts are not guaranteed and will depend on the total number of valid claims filed.
