Claim forms were mailed to merchants containing a Claimant ID and Control Number, yet some eligible business owners say they haven’t received one. If you do not have a Claimant ID and Control Number, you can provide your Taxpayer Identification Number along with some additional information, and the Class Administrator will attempt to determine whether you are a merchant that is eligible to file a claim in this class action settlement.

UPDATED MAY 14, 2024: In an Order dated May 14, 2024, the Court granted an extension of the claims-filing deadline. The deadline to submit claims is August 30, 2024.
